Accidents and Incidents Analysis

The registration of gas accidents and incidents is also an important task for us. The records we keep are used to carry out annual trend analyses, the results of which are reported to the Dutch network operators.
Kiwa Technology also arranges the publication of the records for the purpose of sharing knowledge on safety and to provide additional guarantees for the reliability of the Dutch gas network.
Kiwa Technology investigates the technical causes of gas incidents and the underlying (organisational) reasons.
- We possess extensive laboratory facilities as well as the expertise and experience required for carrying out in-depth technical research of components and materials from gas pipelines and installations.
- We use the Tripod-B system for analysing barrier failures, circumstances and the primary and underlying causes transparently and accurately.
- Our hands-on experience enables us to conduct interviews effectively with people from all layers of the organisations concerned. Our recommendations are underpinned by theory and pragmatic by nature.
- The 24-hour call-out service is available day and night for investigating incidents
- If investigation is required, we analyse whether the accident/incident is caused by the actions of the network operator first.
- If necessary, we carry out a follow-up investigation of the technical and/or organisational causes of the incident.
The aim of incident investigation
For network operators, the aim of investigating incidents focuses on learning from accidents, not on finding out who is to blame. Kiwa Technology also carries out incident investigations, damage analyses and second opinion evaluations on request for the competent authorities (e.g. the police), industrial companies and insurance companies.

Risk Based Asset Management
Kiwa Technology can offer support to operators of pipelines such as gas distribution companies, water boards and heat energy companies when implementing RBAM.
Explosion Safety
Any company where flammable substances are transported, stored and used is required under occupational health and safety legislation to have a policy in place to protect employees against explosion risks. Kiwa has many years of experience in the field of gas and therefore possess in-depth knowledge of the properties of different gases, their various applications, and the associated risks.
Gas distribution safety
Gas distribution safety is currently a topic of high social interest. Organisations such as the supervisory bodies, the Dutch Energy Council and the Dutch National Mines Inspectorate explicitly hold network operators accountable for their responsibilities in ensuring safe gas distribution.
Gasstations and ATEX
Kiwa has extensive expertise in explosion safety, gas infrastructure and regulatory compliance within ATEX environments. We support network operators in assessing, improving and demonstrating compliance for gas installations and gas stations, combining technical knowledge with practical implementation experience.
Risk Management and Analyses
The added value of Risk Based Asset Management (e.g. according to PAS55 or NTA 8120) can be broken down into a number of internal and external benefits.
Gas systems in residential complexes
As a building owner, you are responsible for the safe use of gas. It is therefore essential to have clear insight into the potential risks and the extent of those risks. Kiwa can support you in this. We have developed a method to determine the risk level of a residential complex. Based on the results of this risk analysis, housing associations or homeowners’ associations (Dutch: VVE’s) can identify which complexes require immediate action to prevent unsafe situations. It also provides insight into which complexes present limited risks and shows how simple measures can improve safety levels.
Getting a grip on gas
Actively keeping track of the complete design, installation and management cycle for gas systems owned by housing associations is often difficult by the involvement of many different parties. As a result, housing associations can lose control. Kiwa Technology can help housing associations to regain full control.
Investigating accidents involving gas
A malfunction or defect in a gas installation or one of its components can have major consequences. These may include material damage, financial losses or in the most severe cases, personal injury. Careful investigation of such incidents is essential to determine the cause, prevent recurrence and establish liability. Kiwa supports clients with independent and technically in-depth investigations into accidents and failures involving gas installations.
